Only use this on-line service if:

  • you have a valid and current e-mail address.
  • no more than 10 items of property are damaged.
  • the value of property damaged is under $5000.
  • you are NOT able to nominate a suspect
  • you do NOT require a report number immediately.
  • the damage was NOT due to any type of accident including a traffic crash.
