Traffic History

The Department of Transport and Main Roads (DTMR) is the custodian of documents regarding your traffic and licence history.

Who Can Obtain Information?

Applications for a release of information can only be made if you:

• are requesting your own information
• have been authorised by the licence holder to obtain their information
• have a proven legal authority to obtain the information.

What Information Can You Apply For?

You may apply for:

your driver licence, industry authority, marine licence indicator or adult proof of age card information
your traffic history
your marine licence information
a copy of a document that was given by you to DTMR.

How Do You Apply?

If you wish to obtain a copy of your traffic or licence history, you must complete a Release of Information Application Form (available from http://www.support.transport.qld.gov.au/qt/formsdat.nsf/forms/QF2121/$file/F2121_ES.pdf). You may then lodge the form in person at a DTMR customer service centre or by posting to:
