When to request

  • You have deleted or lost the original e-Ticket Traffic Infringement Notice
  • You did not receive the original e-Ticket Traffic Infringement Notice

What you need to report

  • Personal details including identification, full name and date of birth
  • Email address and contact telephone number
  • Date and approximate location where the traffic infringement notice was issued
  • Vehicle registration, make, model and colour

What to expect

On form submission you will receive a case number. This is not the Infringement Notice number, the case number advises that your form has been successfully submitted.

Subject to operational requirements, you will receive a reply to your nominated email address within 48 hours which will contain a copy of the original e-ticket infringement notice issued.
