​​​​Person History (Record of all Charges in Queensland)

A Person History is a record of all charges that have been preferred against a person by officers of the Queensland Police Service, irrespective of the court outcomes.
It also contains Queensland cautions, community conferences and youth justice agreements under the Youth Justice Act 1992.
It does not contain charges that have been preferred outside of Queensland.
A Person History is only provided to the person to whom it relates. A person wishing to apply for their Person History must make a written request to the Manager Police Information Centre, providing their full name, any previous names, their date and place of birth and brief details of their offender history. Applicants are required to provide a copy of sufficient identification such as a current driver’s licence, passport or other photographic identity document Payment by cheque or money/postal order, made out to the Commissioner of Police, is required at lodgment.​
Refer to the Police Certificate​ section if an Australia-wide criminal history check is required for employment and other purposes.
