Only use this form to apply for a Police Review of a Police Protection Direction (PPD) issued by a police officer. The legislation does not permit police to conduct a Police Review in relation to a Police Protection Notice (PPN).

An application for a Police Review can only be made within 28 days of the PPD Grounds being served on the respondent, or a longer period subject to agreement of the QPS.
