  • What supporting documents are required?

    Throughout the recruitment process, you will be required to provide a number of Supporting Documents (SDs).

    Some of the SDs templates will be emailed to you, whilst others are accessible via this page. You are encouraged to familiarise yourself with the SDs, however please do not complete or submit any documents until requested, as they may change. 

  • SDA - Due at Time of Application Submission

    The following documents are required to be uploaded within your online application form:

    • Driver Licence (certified) - ensure your licence is valid and accurately reflects your residential address.
    • QPSea (ACER) Statement of Results OR approved exemption (certified).

    The following additional documents will be required for applicants applying with policing experience:

    • Resume
    • Policing Experience Documentation (for example, current position description, training records) 

    The following are only required for applicants applying through the International Recruitment Pathway:

    SD1 - Due before Psychological Assessment

    The following documentation will be requested after your Panel Interview:

    • Employment History Details Form
    • Year 12 Certificate (if required) 
    • Birth Certificate (certified)
    • Change of Name Documentation (if required) (certified)
    • Proof of Citizenship or Permanent Residency (certified)

    SD2 - Due before Integrity Vetting

    The following information will be requested through the online Recruit Screening Integrity Form and will be considered as part of your initial integrity assessment. You may be required to provide additional supporting documentation (statutory declarations*, additional consent waivers, etc.) depending on your circumstances:

    SD3 - Due at Pre-Employment Medical Screening (PEMS)

    The following documents will be requested as part of your medical assessment by a QPS approved medical provider: 

    • Health Assessment Questionnaire
    • Specialist Reports (if required) - any additional information/documentation from your medical practitioner or relevant health professional (e.g. psychiatrist, endocrinologist, physiotherapist) regarding any physical or mental health condition/s that are being/have been treated, managed, or monitored. Any clearance documentation needs to confirm that there is no increased risk of re-injury/relapse/future exacerbation of symptoms.

    SD4 - Due at Application & Referee Checks

    The following documentation is required to be completed prior to the Application & Referee Checks stage of the recruitment process:

    SD5 - Due at Allocation & Offer

    Information and documentation relating to your allocation and posting will be required prior to receiving an offer of appointment:

    • Allocation Supporting Documentation (if required - to support your allocation/posting preferences, including extenuating circumstances)

    SD6 - Due at Visa Stage

    This information is required of applicants applying through the International Recruitment Pathway at time of Visa nomination submission.

  • When do you supply your Supporting documents?

    Supporting Documents (SDs) will be requested at various stages in the recruitment process. DO NOT provide any documentation until it is requested. Depending on the circumstances, additional information may be required.

  • What are Certified copies?

    Throughout the recruitment process, you will be required to upload certified/notarised copies of personal identifying documents.

    Copies of documents can be certified/notarised by Qualified Witnesses, as listed on the Queensland Government website, which include:

    • Justice of the Peace* (JP),
    • Commissioner for Declarations* (Cdec),
    • Barrister/Lawyer/Solicitor, and
    • Police Officer.

    Please note that for the certification/notarisation to be valid, it must comply with the standards outlined in the Commissioners for Declarations Handbook (i.e. including certifier's name, signature, occupation, date of certification, and statement of certification).

    *If you are residing within Queensland, you can click here to search for your nearest JP or Cdec.
