Withdrawal of Consent Electronic Document Service

Only use this form if you want to withdraw your consent to receive electronic service of documents by  Queensland Police Service.

What you need 

• Identification e.g. Australian/Queensland Drivers Licence/18+ card, passport
• A valid and current personal email address 

What to expect 

You will receive an online reference number to confirm the form has been submitted. 

Subject to operation requirements, you will receive a confirmation email to the nominated email address within  24 hours.
