Change of Personal Details Notification Form

Use this notification form to note your change of sex and/or name in the Queensland Police Records and Information Management Exchange (QPRIME) computer system. Upon submission of this form, with the accompanying documentation, you will receive a confirmation that your email has been received, followed by a notification when your details have been updated in QPRIME.

Before lodging this notification form, you will need a certified copy in PDF format of one (1) of the below documents:

  • A birth certificate showing affirmed sex (identified gender marker) as issued by an Australian Births, Deaths and Marriages Registry
  • A Recognised Details Certificate from an Australian Births, Deaths and Marriages Registry showing affirmed sex (Identified gender marker)
  • Change of name certificate
  • Passport

To assist the QPS to build an understanding and trust between members of the Trans, Gender Diverse and Non-Binary communities, if you wish to include how you identify please indicate in the field below.

If this field is left blank, the QPS will update your QPRIME information based on the documentation you have provided.
