What you may need

  • Identification e.g. Australian/Queensland Drivers Licence/18+ card, passport
  • A valid and current personal email address
  • Details of vehicle/bicycle involved in the incident

What to expect

You will receive an online reference number to confirm the form has been submitted.

The information you provide will be sent to the police station closest to where the incident occurred. The Queensland Police Service uses this information to understand local traffic behaviour, develop road policing strategies, and investigate traffic-related offences. While you may not always be contacted for further details, your report is valuable and helps improve community safety.

The Queensland Police Service encourages you to confidentially provide your details. You are not required to provide your details however this will impact our ability to investigate and commence prosecution against nominated individuals. If an investigation is commenced, police may contact you to obtain a statement. Police will provide you with further information if you are required to attend court.

If you provide your contact details, you will receive a Qld Police report number in an email or SMS within 5 days.

Photographs or videos

To assist the Queensland Police Service with its investigations, evidence is requested to be provided to the Axon Queensland Police Service Evidence Submission Portal. Providing video or photographic evidence is a valuable source of information to assist police investigations.

For offences reported online with available evidence (e.g. video footage) a secure link will be sent to provide this to Police.

You will receive either an email/SMS with the report number and a secure link to upload the video footage or evidence.

The link can be used:

  • only for the offence being reported,
  • only once to upload multiple attachments and
  • will expire in 10 days once received

If your link expires before you have submitted the evidence, please complete the 'Report Updates' online form to request a new link.

  • Reckless driving
