An application for a Queensland criminal history must be made by the individual concerned. The criminal history is only released to the individual concerned, or to their legal representative with the consent of the individual concerned. This document is most often sought for court-related purposes.
 
The criminal history supplied will be from the Queensland Police Service records only. It does not contain convictions imposed outside of Queensland. A criminal history contains convictions for criminal offences arising from prosecution action initiated by officers of the Queensland Police Service.
 
Persons who seek their criminal history must make application at any police station in Queensland. Applicants can complete the application form prior to lodging it at a police station - Application for a Copy of own Queensland Criminal History​
 
International residents requiring their own Queensland criminal history can obtain the relevant application form by contacting the Police Information Centre, preferably by email. Once the application form is received, the applicant is required to attend their local police station, with appropriate identification, to have their identification verified. Applicants are then to forward the completed application form, along with a copy the photographic identity document to the Manager, Police Information Centre. Payment by personal cheque, bank cheque or money order made out to the Commissioner for Police, is required at lodgment.
