Only use this form if you are reporting on behalf of an alarm monitoring company and need to notify Queensland Police of an alarm activation within Queensland, Australia.

What you need

  • Company details (name, address, contact number)
  • Informants details (full name, position in company, direct contact number [not a 1300 or 1800 number])
  • Keyholders details (full name, direct contact number)
  • Details of contact made (or attempted) with the keyholder regarding this activation

What to expect

You will receive an online reference number to confirm the form has been submitted. This number should be retained if you need to contact Policelink on 131 444 in relation to this incident.

Report alarm activation

This form can be used to report the following alarm types (all other monitored alarm types should be called through to triple zero (000)):

  • Intruder alarms where alarm company is not on site and no CCTV confirmation of offence in progress
  • Communications failure alarms where the only information is that the power has failed or that the alarm is sounding
