Reporting a sexual assault

Before making an online report, please read the information below to see if this option is right for you.

This online form can be used to report sexual assault if:

  • It occurred in Queensland, Australia
  • You are over the age of 18 years
  • You are the victim-survivor or you are reporting on behalf of the victim-survivor with their knowledge and consent

What you need:

  • A current personal email address
  • Authority if you are reporting this matter on behalf of the victim-survivor

What to expect:

This form asks for detailed information about the circumstances before, during, and after the assault. This is not aimed to be intrusive - the form is specifically designed to help police gather as much information as possible.

If you don’t remember or know something, that is okay - just provide what you can. You can take a break or contact a support person if you need to. Your information will only be used for its intended purpose and may be shared with another agency (eg: Office of the Director of Public Prosecutions) if the matter goes to court.

After submitting, you will receive an online reference number – keep this in case you need to contact Policelink (131 444) about the incident. If you opt for a response by email or SMS, you’ll receive a QPS report number within 24 hours (subject to operational demands).

To report sexual assault or harm involving a child under 16  years of age, please call Policelink (131 444) or visit your local police station – please do not use this form. Are you making a mandatory report i.e. under the Child Protection Act 1999?

Do not use this form to make a mandatory report. Mandatory reports relating to sexual abuse are only to be made to police as per your organisation's policy (e.g. by phoning Policelink on 131 444 or using your organisation's online portal).

Did the assault you wish to report occur in Queensland?
